Breast reconstruction with latissimus dorsi flap

Before breast reconstruction by large dorsal lamb with prosthesis

After mammary reconstruction by large dorsal flap with prosthesis
Result at 1 year. Large dorsal torch and anatomical prosthesis of 240 grs.
When the skin is damaged by radiation and its flexibility is insufficient for the introduction of a prosthesis, a musculocutaneous flap of latissimus dorsi is most often used.
It can be used alone or most commonly combined with a prosthesis.
This simple flap construction is widely used in plastic surgery.
A vertical scar is hidden under the arm or a horizontal scar which is hidden under the bra is made to collect the latissimus dorsi flap.
This scar from the removal of the latissimus dorsi muscle may be extended when the amount of skin removed at the back is more than 10 cm wide.
The sequelae of shoulder mobility are minimal or nonexistent.
The impact on working life is negligible, sporting activities can be maintained.
The latissimus dorsi flap, with or without prosthesis can provide well-vascularized tissue at the mastectomy area and allows a good quality reconstruction.
The flap is technically easy, and is widely used in breast reconstruction.
In some cases, to reconstruct the breast size a prosthesis can be used alone, if the opposite side is of a large size with a broad base and ptosis, then a latissimus dorsi flap would not suffice to reconstruct the breast amputated.
In this case, if the abdominal anatomical conditions permit, a flap of the lower abdomen can be proposed.

Before breast reconstruction Complete lack of flexibility at the mastectomy scar. Prior radiotherapy.

After breast reconstruction Reconstruction with latissimus dorsi flap plus anatomical silicone gel prosthesis. Outcome at 1 month

After breast reconstruction with latissimus dorsi flap Final result with reconstruction of the areola and nipple. Scar of the vertical flap concealed under the arm.

Before breast reconstruction Breast cancer recurrence on the left extended to the skin.

After breast reconstruction with latissimus dorsi flap without prosthesis. We expected histological results to finish the reconstruction.

After breast reconstruction with implant and symmetrization of the right breast.

After breast reconstruction
Final outcome at 1 year.
Vertical scar at the back barely visible and hidden under the arm.

Before breast reconstrcution
Lack of scar flexibility.
History of radiotherapy.

After breast reconstruction by latissimus dorsi flap with prosthesis.
Outcome at 1 month.
Angular scar at the back.

After breast reconstruction
Final outcome at 6 months with left breast reduction.

After breast reconstruction by latissimus dorsi flap.
Final outcome at 3 months and at 18 months.
The scars will fade with time.

Before breast reconstruction by latissimus dorsi flap.
After breast reconstruction by latissimus dorsi flap with a prosthesis. Result in a year.
